An accident is as an unplanned, unwanted event resulting in a loss. Loss is not confined to personal injury, other examples include loss of business, revenue, opportunity and reputation.

A near miss is as an unplanned, unwanted event that has the potential to result in loss. It is important that any near miss is reported and escalated internally for investigation, to prevent a reoccurrence.

It's helpful to think of the events leading up to an accident as a row of dominoes: it helps to explain accident causation.

A - Lack of management control (root cause)
B - Individual and job factors (basic causes)
C - Unsafe acts and/or conditions (immediate causes)
D - Accident
E - Loss
(Underlying causes = A and B)

Understanding the domino theory and acting on points A - C will better equip your organisation and significantly reduce the likelihood of an accident occurring.

How can we help?

We provide a range of services, based on your needs, from telephone assistance following an accident at work, to a plan or photographs of the site of the accident up to a full accident at work investigative service detailing the circumstances and causation of an accident at work.

Upon your instruction we will visit the site and begin to gather and analyse all relevant information, we will conduct the necessary interviews and obtain statements from witnesses. We will draw conclusions based on evidential facts, and where necessary, make appropriate recommendations.

We aim to provide you with the underlying and immediate causes as well as liability. Our comprehensive reports, unless otherwise instructed, will include the following information.

A general overview

To give a brief explanation of the incident, highlighting major information only to give a clear indication of who was involved and how the accident occurred.

Attending the workplace - Site analysis

To enable the recipient to visualise the site of the accident and what occurred from the information given.

A detailed plan of the Site

To enable the recipient to gain as clear a picture as possible of the site, and will include:
• A clear an uncomplicated diagram of the site.
• Measurements of the immediate area and any other key factors.
• Numbered Photo positions - including measurements.

Photographic images/Photo list and photo analysis

Each photo will clearly state where the picture was taken, what of, and, if relevant, the direction it faces, and the perspective it was taken from, eg, the employees view, or what it shows, e.g. audible warning lights.

Photographs will be clear and as true a representation as possible, numbered to correspond with the photo list.

Statement Analysis

Providing the salient points of witness testimony which aid to establishing facts in helping to establish what happened.

Report Conclusion and Recommends

To highlight the evidence and provide you with the underlying and immediate causes of the accident, as well as liability.

Our NEBOSH qualified investigator will make appropriate short, medium and long term recommends, if you prefer, we can also conduct a post-accident risk assessment for you, this can be discussed once the final report has been submitted.
